Home Health Services

Inland Home Health Providers believes that everyone is entitled to quality healthcare. Our staff and officers are highly trained professionals who share this belief and use their skills to provide the best care to their patients. 

Home health care is part of the health care delivery program to provide medical attention on an intermittent basis. It is available for patients after they are discharged from the hospital or other health facilities, especially to those who have had recent changes in medications, treatment plans, or health status and need help to recover in their own home.

Professional Care

We send the following health care professionals to your home to provide services:
Registered Nurses (RN) - provide thorough and comprehensive assessments to identify home health needs and develop a plan of treatment including teaching programs to enhance knowledge and independence in the home, and promote maximum recovery.

Licensed Vocational Nurses (LVN) - under the supervision of RN and physician, provide treatment and education based on patients needs, ongoing evaluation of patient’s health status. 

Certified Home Health Aides (CHHA) – provide assistance with activities of daily living such as bathing and dressing.

Physical Therapists (PT) - evaluate and treat patients who suffer from pain, weakness, limitation of movement, difficulty in transfers and gait abnormalities through therapeutic exercises, gait training, prosthetic training and other home safety programs.

Occupational Therapists (OT) – evaluate and treat patients with limited ability to complete activities of daily
living through therapeutic exercises. 

Speech Therapists (ST) - help patients improve their speaking and swallowing abilities.

Medical Social Workers (MSW) - help patients and family members understand and cope with problems caused by illness or injury and provide access to available community resources such as transportation and caregiving resources.

Durable Medical Equipment (DME)

Inland Home Health Providers staff make it easy for your loved ones to receive the medical equipment they need to be safe and independent at home. We order the supplies and arrange for delivery to your home. Below is just a sample of the few types of equipment we help you receive.

Payment

Payment for the home health services is covered by the patient's insurance. Inland Home Health Providers will settle payment directly with the insurance carrier. If for any reason a patient's insurance is inadequate to cover all charges, we will inform you in advance to make other arrangements.

Hours of Operations

Our office hours are from 8:30 a.m. to 5:00 p.m., Monday to Friday and skilled services are available seven days a week. We also have on-call nurses available to receive your call after office hours, on weekends, and holidays to help you.
